Step by step guide to the Schengen application form

Each Schengen country may produce its own variant of the standard Schengen visa application form.  However, the application forms do not vary a great deal and completing the visa application form is generally straightforward.

Below is a section-by-section guide to filling out a standard Schengen visa application form.  An example of a correctly filled-out application form is also available in Appendix C.

If you are living in a marriage-like relationship with your partner without being legally married, tick "other" and enter "unmarried partner".  If you are in a civil partnership or PACS with a French citizen, tick "other" and  write

Enter details regarding the child's legal guardian

National insurance (NI) number
Leave it blank unless you have resided in the UK and have been issued a NI number

Tick "National Passport" if you hold an ordinary passport

If you are residing in the UK, tick "Yes" and enter your UK visa number.  This is located in the UK visa in your passport, on the top right hand corner.  It is either a 9 digit number (e.g. 123456789), or you may have a residence permit stamp with a black 7-digit number preceded by UK (e.g. UK1234567).  Enter also your Expiry Date if applicable

Enter your current occupation (e.g. doctor, student, engineer)

Employer and employer's address
For workers, enter the name, address and telephone number of your employer. 

For students, enter the name, address and telephone number of your education institution

Main purpose(s) of the journey
Tick as appropriate

Member State(s) of destination
Enter the Schengen countries that you will be travelling to (e.g. France and Italy)

Member State of first country
Enter the Schengen country where you will enter first

Number of entries
Tick as appropriate.  Note if you are traveling to more than one Schengen country, you must tick "Multiple"

Duration of intended stay
State the number of days you wish the visa to be valid for.  This cannot be longer than 6 months, nor longer than your UK visa validity

Enter details of previous Schengen visa(s) granted (e.g. Germany 2009)

Tick as appropriate and enter details if your fingerprints were taken in order to grant you a Schengen visa

Only complete if applying for a transit visa.

Complete only if applying for a business Schengen visa.  Enter details (name, address, telephone number) of the company that has invited you

  2. Your wife should sign it. Although you should also write and sign separately a statement stating your income and how you will support her financially throughout the duration that she will be in the Schengen zone.

    1. This is incorrect. Under EU law there is NO requirement to show sufficient funds for either the EU citizen or the NON-EU spouse of the EU citizen, notwithstanding that some EU countries are asking for this. Nor is there any need to show hotel bookings.

  3. What if i dont have a surname? And my passport only contains Given Names. What do i put in the surname section then, Because without it, the online form is still in pending state.

  5. What if I have booked three hotels and only one hotel is pre-paid. Other two hotels will be paid at the arrival? How am I going to answer Q33? Is it okey to pay the hotel in the arrival?

  9. Thank you so much for this blog, it is so helpful! I will be visiting Italy only and will be a tourist staying in hotels, no individual or company invited me. I was very confused about what to do with question 32 and the sub-questions having to do with company/organization and contact person within that company. You say to leave that blank. You don't think they want me to repeat the information for the hotels and provide a contact name at each hotel? Thanks!

    I am an Indian living in USA working on what we call as H1B visa. Currently I work with a different company but previously when I got my USA H1b Visa stamped on my passport in India it was with other company however I have the document called I797 that says I have a valid visa for the current company in USA so when I apply for Schengen visa what Visa number should I write ? The one on the passport with my previous company stamped or the new number I got in the document because of my new company ?

  14. Your answer to question 24 is wrong. This question does not relate to how many Shengen countries you will visit but how many entries you want INTO the shengen zone. Once in the Shengen zone you can visit as many countries as you like on a single entry visa as long as you don't leave the zone.

    If you do and then want to return you need either a double or multiple entry visa (or apply again). So, for example, if you enter the Netherlands from the UK then go back to the UK and then want to enter Belgium you would apply for a double entry.

    I am travelling to Paris then to UK. However I still have not obtain my UK visit visa yet.
    I am stuck in question 28

    "Entry permit for the final country of destination"

    What should I answer in this case? I am afraid that I will not get my schengen visa because of this issue.

    Also with the airticket booking, could you kindly recommend whether there is any website that could make valid reservation for the flights.
